AndyB - Attachment browser 1.4

Remove no longer used code in a template.

Now supports sites which don't have Use full friendly URLs enabled.

Attachment browser link move to tab header as default. Options page has option to add Attachment browser button next to the Find button.

Pages now load almost instantly.
Fixed Attachment browser button location on mobile devices.
